MDS NURSE MANAGER
Job Summary: The Piatt County Nursing Home, a 100-bed skilled nursing facility located in Monticello, Illinois, currently has a full-time opportunity for an experienced MDS Coordinator Nurse Manager. The MDS Coordinator position will have a current RN or LPN license. The position of MDS Nurse Coordinator ensures a coordination of quality care to residents, from pre-admission through discharge. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement. The MDS Nurse Coordinator manages the resources provided to residents and coordinates the entire continuum of care. Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. Piatt County Nursing home offers IMRF to employees. This position reports to the Director of Nursing.

Job Expectations:

RN or LPN with current Illinois nursing license in good standing
Recent experience as an MDS coordinator in a skilled nursing facility
Excellent assessment and clinical skills
Should be familiar with all aspects of MDS including the assessments, submission requirements, reporting, care plans, care plan meetings
Must be able to work in collaboration with the interdisciplinary team in assuring that the MDS is completed timely
Should be computer literate
Supervise and manage nursing floor staff and assist floor nurses
Admissions/Re-Admissions along with admission screenings.
Ensure compliance nursing care standards and long-term care regulations.
Review and follow up on labs and processing orders.
Communicate with Doctors and POAs with updates and entering physician visits in Point-Click Care.

Pay: $35.00 - $45.00 per hour
Expected hours: 40 per week
